The Foundation refers to the order of the Ministry of Health of the Russian Federation on the procedure for planned hospitalization in a pandemic, according to which all patients who are indicated for inpatient treatment must undergo a nucleic acid amplification (PCR) analysis. A laboratory study of the analysis should be performed no earlier than 7 days before admission to the hospital, the MHIF specifies.
In addition, immediately before hospitalization, a general practitioner or pediatrician must conduct a non-contact measurement of body temperature and pulse oximetry, which indicates the level of oxygen in the blood. In the presence of an elevated temperature, low oxygen level and other indications, the doctor may send the patient for an X-ray or computed tomography of the lungs, as well as for an additional study for COVID-19, including using an express test.
Elena Chernyakova, Chairman of the Federal MHI Fund, stressed that all examinations are carried out only at the expense of the MHI funds, that is, with a medical policy. Therefore, any requirement from a medical institution to make a test during a planned hospitalization at its own expense is a violation. In this case, the MHIF recommends contacting the insurance organization that issued the MHI policy.
The Foundation also recalled that the Ministry of Health established the procedure for hospitalizing patients with confirmed COVID-19, depending on the severity of the disease and the order of discharge.
According to the changes, patients with coronavirus who have a temperature of 38.5 ° C for 3 days and a blood oxygen saturation of less than 95% are subject to hospitalization, as well as signs of pneumonia with a prevalence of changes in both lungs of more than 25% based on CT results.
Hospitalization is also subject to patients over 65 years of age with severe chronic diseases: arterial hypertension, chronic heart failure, oncological diseases, diabetes mellitus, rheumatoid arthritis. Also, inpatient treatment is indicated for patients with COVID-19 who need hemodialysis, undergo chemotherapy, or have immunodeficiency states.
In addition, people with COVID-19 who live in a hostel, a crowded apartment or with people over 65 and at risk can be hospitalized.
Children under 18 years of age with coronavirus infection are shown hospital treatment if the temperature rises above 39.0 ° C on the day of treatment or stays above 38 ° C for 5 days, with saturation less than 95%, signs of respiratory failure, tachycardia, shortness of breath, episodes of apnea, the presence of hemorrhagic rash, etc. The hospital should also treat children with severe concomitant diseases: immunodeficiency state, cancer, heart defects, diabetes, obesity, etc. A referral to a hospital should be issued to a child who lives with relatives, belonging to the risk group and in the event that he does not have the conditions for fulfilling the recommendations at home.
According to the order, the transportation of the patient upon discharge or transfer to another hospital for follow-up care is carried out in an ambulance, while the driver and accompanying medical worker must be provided with PPE - a type 1 anti-plague suit or a disposable gown, glasses, respirator, gloves, shoe covers.
